How to Claim

Allianz - Canadian Traveler

• When Hospital (or medical clinic) has a Direct payment arrangement with Allianz  

  • ** You can ask Allianz for its designated hospital for direct payment closest to you or you can also find the list from here: Direct Pay Arranged Facilities
  •  
  • 1. Before you go to medical clinic, or when at the clinic (some hospital will make a call for you, but this depends on their willingness), make a call to Allianz claim department at 1-800-995-1662. Press 1(English) then press 1(Medical Emergency) 
  • 2. When you get connected with a live Allianz agent, state your policy number with your full name to open a claim. (Policy number can be found on Health card or insurance confirmation paper)
  • 3. Provide brief explanation of why you are going to, or you are at, hospital.
  • 4. Now that you have successfully opened the claim, Allianz will send a Provider letter to hospital(or medical clinic) to facilitate direct payment process. The Provider Letter contains your insurance coverage details along with your claim number, and it usually takes 30 min to an hour for hospital(or medical clinic) to receive it.
  • 5. When you are back home from medical clinic, you still need to submit a Claim Form. This will facilitate a direct payment from Allianz to hospital(or medical clinic) for any future medical cost as well as extra medical expenses(such as prescriptions, medical devices) which you may have paid to pharmacy, related to this claim.
  • 6. To get reimbursed for the expenses out of your pocket, please follow instructions shown on Claim Form. Send the form via regular mail along with all the original invoices (you should keep a copy of the invoices for your records) and proof of payments.

* Special notes:
We understand how hard it may be to call claim department, especially when you are in a medical emergency situation. The claim process may seem inconvenient and uncomfortable. Reporting your claim to Insurance company as early as possible, however, will help you proceed through the claim process quicker. Unlike Canadian provincial health plan holder, most travelers or temporary visitors in Canada do not have their medical records in provincial medical data base. As such, your communication with insurance company is the only way to ensure that all the medical cost be covered by insurance company.

  • Remember the following to make your claim experience smooth.
  • Update your current address and phone number to Allianz when you call
  • When you are on your way to hospital emergency room, please try to place a call with Allianz as soon as possible and follow the instructions. This will ensure that all the cost be paid by Allianz directly to hospital.
  • A signed Claim Form submission is very important because Allianz will need your authorization to discuss your claim with hospital. Please make sure to submit the form as soon as possible.

• Getting reimbursement from Allianz after you paid hospital(or medical clinic)

  • 1. Before you go to medical clinic, or when at the clinic (some hospital will make a call for you but this depends on their willingness), make a call to Allianz claim department at 1-800-995-1662. Press 1(English) then press 1(Medical Emergency). If contacting Allianz after the visit to hospital or medical clinic, this should be made within 30 days of occurrence.
  • 2. When you get connected with a live Allianz agent, state your policy number with your full name to open a claim (Policy number can be found on Health card or insurance confirmation paper)
  • 3. Provide brief explanation of why you are going to, or you are at, hospital.
  • 4. When a claim is successfully open, Allianz will send a Claim Form to your updated home address to start the process of expense reimbursement. Or you can download Claim Form.
  • 5. Please follow instructions shown on Claim Form. Send the form via regular mail along with all the original invoices (you should keep a copy of invoices for your record) and proof of payments.
  • 6. You have 90 days to send all the related document along with Claim Form to Allianz.
  • 7. It typically takes 2 to 4 weeks to process the payment. You can send email to claims.to@allianz-assistance.ca to check the status of your claim using the claim number assigned to you at the time of initial call.
